Let’s say you’re getting fuel and notice on a neighboring truck an issue that could cause an issue (up to death) for the driver and or others on the road.
What would you do?
Why?

Chose #3 because that's what we usually do around here.Let’s say you’re getting fuel and notice on a neighboring truck an issue that could cause an issue (up to death) for the driver and or others on the road.

So you wouldn’t say anything to them at all?Chose #3 because that's what we usually do around here.
They weren’t meant as an escalating scale but as what action do you take.Three of your four options are kind of an escalating scale.
First thing is you approached the driver and pointed out, and his response dictates what happens next.
If he blows you off? Shaming is fair game, unless it's something that is an obvious threat to life and limb, say an obviously failed bearing, a severely broken wheel, or freight that's going to fail off, etc. In that case, he deserves a dime dropped on him. And screw him for not giving a crap about those of us around him.
If it's obvious the driver didn't know, and is going to take steps to rectify, they're generally getting a pass. No post, no dime, a helping hand if needed (within reason. )

I would let them know because I am a good fucking person. (I know the F-word was not required)
I would like someone to do the same for me and they have.
When I was a rookie I got a temp job at a dairy products plant. So of course it was a reefer (straight truck) the fuel line for the reefer came loose and was on the exhaust pipe. Some nice driver pointed it out to me and even zip-tied it up. I will never forget that!
I was being a "smart-ass" okay.So you wouldn’t say anything to them at all?
Are you afraid of them?
That is the thing these days, Ya never know what kind of reaction you are going to get.Three of your four options are kind of an escalating scale.

I suppose an extra option could've been take a picture and notify the companys safety office with unit number and location let them talk to the driverThat is the thing these days, Ya never know what kind of reaction you are going to get.
Years ago, guy passed me with pallet rack frames. I hollered on the radio that one was on the verge of slipping out of the stack. That is the thing these days, Ya never know what kind of reaction you are going to get.
This is the Way...I was stuck in an orange barrel induced backup on I-65 north of Indy once.
White Volvo Mafia had his trailer's underbelly air lines dragging on the because his tandems were all the way forward and that spring was broken.
When I got next to him I set the brakes & leaned out the passenger window and told him.
He said "Yeesh Yeesh, I feex, thank you" " and he got out right there in the right lane and put zip ties on it.
Traffic never moved an inch until he was done and back in the can.
at least 3 times in my entire career, i had seen something and made the driver(s) aware. each damned time, i was told to don't worry about it, or mind my own business.Let’s say you’re getting fuel and notice on a neighboring truck an issue that could cause an issue (up to death) for the driver and or others on the road.
What would you do?

I'd tell the drivers Ma & Dad then they'd be in really big trouble when they got homeI’d tell the driver
This.I have and will go up to them and let them know.
I would want someone to do the same for me,
