Let me guess, sidewall blew out for about a foot.

Its called a zipper tear. And it happens from a bad install. You'd never know it unless you were standing close enough to hear it as it inflates. Scary as hell when they come apart on you. Part of the reason OSHA requires tires to be inflated in cages.
Are you sure it's OSHA and not just a handful of insurance companies? Or maybe a state law in MN?

I've seen the cages before but most of the tire shops I've been in don't have them. I was just at one last week. I went there twice. First to have them dismount one of my tractor tires from the rim, then to re-mount it with new inner tube after I welded up the rust holes & painted it.

They do everything from wheelbarrow tires to giant ag tires there. No inflation cage.
 
So why do so few tire shops actually use them? :headscratch2:
